Services we offer
Support Independent Living
Supported Independent Living (SIL) is part of the National Disability Insurance Scheme (NDIS) and provides support for individuals with permanent or significant disabilities, either in their own homes or in shared SIL housing. It includes assistance or supervision with daily tasks such as personal care, meal preparation, and household chores.
SIL is designed for people with higher support needs who require ongoing help, whether it’s with laundry services, cleaning, transportation, or other daily activities. The main goal of SIL is to help individuals live as independently as possible.
Personal Care
Personal care refers to non-medical home support that assists individuals with daily tasks such as bathing, dressing, grooming, mobility, eating, and using the toilet. The level of personal care required can range from a few hours per week to extensive daily assistance. This service is highly individualized. Those who may need personal care include people with disabilities, elderly individuals, people with illnesses, and those experiencing cognitive impairments.
Personal care is a fundamental right for everyone. We feel more confident, comfortable, and presentable when our personal hygiene is well-maintained, especially before going out. Having a dedicated in-home carer for personal hygiene can significantly enhance your daily routine. At Connecting Care, we provide personalized care that is respectful, gentle, and focused on your dignity
Assist-Travel/Transport
We recognize that our clients have different needs when it comes to assistance. For example, you might rely on a family member to drive you to appointments or family visits, but with work and other obligations, that can become challenging.
To help, we offer services designed to make travel easier for you. We can even provide taxi vouchers, allowing you to travel independently. No matter what your specific needs are, we’re here to assist in the way that works best for you. Getting out and about is a part of daily life, whether you’re planning an outing, doing your weekly shopping, or going to a medical appointment. It’s important to feel safe and confident while doing so.
Complex Care Service
Complex care is designed for individuals with higher-level needs, often due to disabilities, chronic conditions, illnesses, or frailty associated with aging. This type of care typically involves a wide range of services, including bed transfers, assistance with daily tasks like toileting and showering, bowel care, tracheostomy management, and managing conditions such as diabetes.
Finding the right complex care providers can be overwhelming, especially if you’re new to accessing support. Government-funded services like the NDIS can help guide you through finding the right support network, enabling you to live as independently as possible and enjoy a better quality of life. If you’re new to this journey, we are here to help.
Daily Living & Life Skills
Daily living and life skills refer to the practical abilities needed to handle day-to-day tasks on your own. This includes personal hygiene, meal preparation, managing finances, using public transport, maintaining communication, keeping your home tidy, and navigating social interactions—essentially, everything needed to live independently in a community.
Community Participation Support
Engaging in social and recreational activities is essential for many, and while some people may enjoy these activities independently, others may need support from friends, family, or the community. If your disability makes it harder for you to participate, the NDIS can help fund the support you need.
As part of the NDIS core support category, any assistance required to access social or community activities is covered nu NDIS. This means that you can have a support worker accompany you to a book club, sports practice, or even a music festival.
Domestic Assistance Service
A clean and safe home is essential for a happy, healthy lifestyle, but maintaining it involves everyday chores like cleaning, laundry, cooking, and dishwashing. If you have a disability, you may need extra support to manage these important tasks each day.
NDIS Funding for Household Task Services:
Help with household chores and domestic tasks is available for NDIS participants through the Core Supports budget, under the Assistance with Daily Life category. This also covers personal care services, such as assistance with showering and dressing. The goal is to help participants gain more independence in their daily lives, enabling them to live in the community and enjoy a higher quality of life.
Nursing Care
The Community Nursing Care service is available to NDIS participants aged 18 and over in Sydney and its surrounding suburbs, including those living at home, in supported housing, or in residential homes.
When dealing with a physical disability or undergoing a transitional physical phase, it’s important to receive care under the supervision of a qualified nurse. Relying on non-professional family members for personalized care may not adequately address your needs and could potentially make things worse.
Group & Centre Based Activities
If you are interested in joining group activities in centres and communities across Australia, you are in the right place. Our centers and community-based programs are designed to foster social connections among people with disabilities, the elderly, and NDIS participants. These programs offer both facilitated and self- directed capacity-building activities. We encourage all our clients to participate independently in the activities, as well as attend workshops organized by us.
Coordination of Support
The National Disability Insurance Scheme (NDIS) was established by the Australian federal government to provide support for individuals living with disabilities, ensuring assistance in various aspects of their lives. With approximately 4.5 million Australians living with some form of disability, the government implemented this scheme to improve their quality of life. An NDIS specialist support coordinator is an expert who uses their knowledge and skills to help individuals with disabilities navigate life in the most independent and self-sufficient way possible.
Our efficient support coordinators and management team are here to help
you navigate the complexities of NDIS terms and services, ensuring you
receive the support you need with ease.
